Care Management is a program for people who need help managing their health. The program’s goal is to ensure you get the right care at the right time in the right place.
How does the program work?
A care manager will contact you to discuss your health needs and goals. He or she then will develop a personalized care plan to share with you and your doctor. Once you have a plan in place, the care manager will follow up with you on a regular basis to see how you’re doing and what help is needed.
What is the care manager’s role?
A care manager is a nurse or social worker who helps coordinate your care. He or she also helps you better understand the healthcare system and removes barriers to care by connecting you with community resources. In addition, the care manager provides support and encouragement to enable you to take charge of your health.
