Spinal Cord Injury Program

Our Spinal Cord Injury Program focuses on the goals of our patients and their families through the outpatient rehabilitation process. Treatment focuses on providing education and facilitating independence in the home and community. The team is comprised of the patient, family and the following team members:

  • Case Management
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Recreational Therapy

Additionally we offer the following specialized services as indicated:

  • Wound care
  • Orthotics
  • Wheelchair Clinic
  • Driver Training
  • Re-Walk™ Training Center
  • Community support through the Breeze SCI Support Group

Entry into the outpatient program begins with a physician referral. When a patient presents for assessment, the interdisciplinary team members are mobilized to begin the evaluation process, the services the patient receives depends on the referrals made to the therapy department based on the patient’s needs.

The team convenes monthly to collaborate and coordinate the goals and assess concerns or obstacles to the established plan. The patient/family’s input is provided through the case manager, family conferences as needed, as well as any member of the team. Referrals to appropriate support agencies are initiated and medical equipment is secured. Post-discharge regimes are established through a collaborative relationship with the patient/family, community services, the family physician, and the follow-up physician visits. A focus is also on facilitating community reintegration through community based recreational therapy referral where life care skills and accessibility needs are evaluated.
