Thank you for choosing Family Medicine–Mountain View to meet your healthcare needs. Our practice treats patients from age 2 years and up, providing services ranging from preventive care to sick care.

Our desire is to offer complete, expert care whenever you and your family need it. Your provider or another Mountain View partner is always on call if you have an after-hours problem. If the problem does not require immediate care and you would prefer to see your provider, we will schedule an appointment for you for the first available time.

Complementing our team is a nurse practitioner who provides comprehensive health care to both adults and children. The nurse practitioner performs health assessments and diagnoses, conducts physical examinations, orders lab work and X-rays, prescribes medication, coordinates treatment plans and makes referrals. She also educates patients and their families to promote wellness and maintain health.

Family Medicine–Mountain View uses electronic medical records, which permit access to your records from many areas of the hospital and from many physician offices, including specialists. This information helps us manage your care efficiently, promptly and securely.

Emergencies

In a life-threatening emergency, call 911 at once. Otherwise, call our office before seeking care. A staff member will handle your call and give you instructions on how to take care of the situation. If you need to speak with a provider after hours, please call (864) 877-9066 for instructions on reaching the on-call physician.

Office Policies

Appointments

Our goal is to provide high-quality care and to see you as quickly as possible. Office hours are 7 a.m.-5 p.m., Monday through Friday. For appointments, please call (864) 877-9066.

For cancellations, please notify us 24 hours in advance if at all possible, thus freeing up your appointment for another patient. If you are a new patient, please arrive 20 minutes before your appointment and bring all of your medication bottles (prescription and over the counter). We also will need a picture ID and your current insurance card. If time permits, we will mail the new patient paperwork to you ahead of time.

We make every effort to schedule your appointments with your provider. However, if your provider is not available and you require immediate attention at our office, your care will be provided by another provider in the practice.

Weekends and After Hours

We are on call 24 hours a day, seven days a week. Call our main number to reach our answering service.

Medical Forms

To protect your confidentiality, we will not release medical information without your written authorization or unless required by law. When you sign a release form, we cannot withhold any medical facts from the people or organizations for which you have authorized release.

For school physicals, athletic exams, overseas travel, preoperative consultations, insurance exams, etc., please complete your portion of the form before coming in. If you send forms to the office for completion by your provider, please enclose a self-addressed, stamped envelope.

Payment Policies and Fees

We request payment for office visits at the time of service. We accept cash, checks and major credit cards. Fees vary depending on the complexity and number of problems, provider’s time, diagnostic tests, medications, immunizations, etc. If you have questions about fees, our office is always willing to discuss them with you.

Insurance

As a courtesy, we routinely file insurance and ask that you bring your current insurance card with you at each visit. Our staff will assist you with any health insurance problems. We participate with several health maintenance (HMO) and Preferred Provider Organizations (PPO). If you are a member of these groups, please call our office to confirm our participation and any special provisions your insurance company may require.

As participating Medicare providers, we file all Medicare charges, collecting only the Medicare deductible and co-payment from you at the time of service. If you have secondary insurance, we will file it for you also. Please keep all invoices that we give you in case paper filing is necessary.

Phone Calls

Phone calls and messages are handled as promptly as possible. Your provider’s nurse/medical assistant typically returns the phone calls. To assist your provider, the assistant can speak with you about your concerns or questions. Please give as much information as you can when you leave your message.

For children, the person most closely involved with the child’s care is the preferred contact. When leaving your message, please include the patient’s overall condition (such as temperature and symptoms) and whether the patient is taking medications. Also, provide the telephone number for your pharmacy in case a prescription is needed.
