Prisma Health Home Recovery Care is a care option that allows you to safely receive treatment for your medical condition at home, instead of the hospital.  Home Recovery Care services are available to our patients at Prisma Health Greenville Memorial Hospital.

Home Recovery Care lasts 30-days to ensure you are recovering. Our services are available 24/7 to Home Recovery Care patients.

What to Expect While in Home Recovery Care (30-days)

First 3-5 Days

Your doctor determines if Home Recovery Care is a good option of care for you. If so, you will have the option to go home to receive your care, instead of being admitted to the hospital. Once you leave the emergency department, your Home Recovery Care services begin. Your Prisma Health doctor will work directly with your assigned registered nurse who will care for you in the comfort of your own home twice a day for 1-3 hours at a time.

All the necessary equipment you might need will be set up for you, such as an IV, oxygen and pulse oximeter. This includes a computer tablet, which allows you to see and talk to your doctor from home and communicate with your care team. The computer tablet is at no extra cost but will be collected by our care team and returned at the end of the 30-days.

Your Registered Nurse Will:

  • Take your vitals, such as blood pressure, temperature and weight.
  • Administer IV medications.
  • Help you take your medications.
  • Assist you with pain management.

You will also have access to a Recovery Care Coordinator. The Recovery Care Coordinator is also a registered nurse and is available to you 24/7. They will be your main point of contact for questions or concerns throughout the 30-day recovery.

Your Recovery Care Coordinator Will:

  • Be available to you 24/7.
  • Work with you and your doctor(s) to arrange all care services delivered right to your home.
  • Coordinate and communicate your care with your doctor(s). This includes your primary care physician and other specialists.
  • Track your vitals daily.
  • Assist in scheduling your follow-up appointments.
  • Provide you with health education.

Last 25-27 Days

After the first few days in Home Recovery Care, your care will transition back to your primary care physician. You will then resume your normal activities.  For the rest of the 30-day period, you will still have access to your Recovery Care Coordinator. They will be available 24/7, should you have questions or concerns. Your Recovery Care Coordinator will help schedule follow-up visits with your primary care doctor and continue to track your progress.
