Caregiver Coaching Program

Prisma Health’s Center for Success in Aging Memory Health Caregiver Coaching Program aims to improve knowledge and support for caregivers, thereby reducing stress and enhancing their ability to care for their loved ones.

Participants will be assigned a Caregiver Coach. The coach will perform an assessment followed by targeted training. The training consists of 4-12 home visits or phone calls over 6-12 months.

Eligible participants include people with dementia and their caregivers who:

  • Live at home or in assisted living
  • Have moderate to severe dementia
  • Have caregivers living with them or separately spending 10+ hrs/wk providing care
  • Have caregivers unable to access community resources who would benefit from home coaching

This program is open to all dementia caregivers and is part of Prisma Health’s Center for Success in Aging. To make a referral, please contact the Center for Success in Aging Memory Health Program at (864) 454-8120

Virtual Dementia Tour

The Virtual Dementia Tour is a scientifically proven method of training designed to build sensitivity and awareness in caregivers by simulating the physical and mental challenges of dementia and Alzheimer’s Disease. This learning opportunity will help caregivers understand how to communicate and assist their loved one more effectively. This experience is offered free of charge and facilitated by the staff of the Memory Health Program. Appointments are scheduled in 15 minute intervals.

Caregiving ABC’s

This series provides helpful information to those caring for loved ones affected by memory health conditions. Each session builds on previous sessions to increase caregivers’ knowledge of maintaining their loved one’s independence.

You may be eligible for an in-home caregiver, at no cost to you, to stay with your loved one while you attend. Learn how to access this service by calling the Alzheimer’s Association at

1-800-272-3900 no less than 2 weeks in advance of the date that a caregiver is needed!
