Geriatric Medicine

The Division of Geriatrics provides medical consultations in both inpatient and outpatient settings. Board certified geriatricians, nurse practitioners, licensed, master’s-prepared social workers, and registered nurses function in an interdisciplinary team. This innovative approach to geriatric medicine is growing and dynamic, and the team provides patient-focused and family-focused care. Prisma Health is developing other programs to meet the special needs of our aging population. The Center for Success in Aging is the headquarters for all activities of the Division of Geriatrics.

Internal medicine residents train with the division, and team members are involved in research and education with partners including Clemson and USC School of Medicine and Public Health.

Inpatient services

Geriatric Medicine consultation services are available at Greenville Memorial Hospital. Geriatric Medicine experts work with a patient’s medical team during his or her hospitalization. To refer a patient for geriatric consultation, please call 864-454-8120.

Consultation services are also provided at multiple Long-Term care facilities.

Outpatient services

The Center for Success in Aging offers patient consultations for individuals with memory and cognitive impairments along with support and education for families and care partners of those individuals.   • Memory Health program – Patients with memory health conditions such as Alzheimer’s Disease and Related Disorders.
  • Caregiving ABCs is a six-session program designed to offer support and advise for those caring for people with memory issues. Lunch-and-learn caregiver series is also offered.

Prisma Health Senior Care (PACE)

PACE (Program of All-inclusive Care for the Elderly) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

Center for Success in Aging—Memory Health Program

The Center for Success in Aging—Memory Health Program offers a comprehensive evaluation which includes medical, cognitive, social and physical evaluations to diagnose memory conditions and develop a plan of care for patients and their family members or care partners.

Memory Health Caregiver Services

Prisma Health’s Center for Success in Aging Memory Health Caregiver Services aim to improve knowledge and support for caregivers, thereby reducing stress and enhancing their ability to care for their loved ones.
