For tens of thousands of people in our community without health insurance, the cost of health care can prevent them from getting care when they need it. AccessHealth is a program that brings together a network of health care providers and community agencies focused on supporting the health and well-being of people who cannot afford to get care on their own.
AccessHealth is not a direct provider of health care services, but works with providers to ensure program participants can get timely, affordable, high quality care. Additionally, AccessHealth is not health insurance and the program does not pay for care. Rather, AccessHealth creates avenues for health care providers and facilities to offer their services at little or no cost to program participants while demonstrating value that impacts the entire community.
Care coordination, navigation and case management support from AccessHealth staff guides participants through the health care system, enabling them to obtain services they need, and empowering them to make good health and lifestyle choices. Healthier people make a healthier community. AccessHealth makes the connections that make it possible.
- Assist participants to establish a relationship with a provider (or a medical home), and receive affordable access to specialists, and other providers
- Help participants understand and manage their health conditions
- Ensure participants can get their medications and can follow their doctor’s instructions
- Address other social barriers like housing, transportation, education and more, which may make it difficult to live a healthy lifestyle
Do You Qualify for Services?
AccessHealth team members screen uninsured patients for enrollment based on household income and residency. If you or someone you know could benefit from enrollment, contact us for an eligibility appointment. Resources vary by county.
AccessHealth serves as the gateway to care for the uninsured in our community. Once enrolled in AccessHealth, participants may also enjoy the support of the Neighborhood Health Partners (NHP) Care Team or the GHS Mobile Health Clinic.
AccessHealth is supported by grants from The Duke Endowment and is one of several networks across North and South Carolina working to ensure lack of insurance and income do not keep people from being healthy.
