If you have questions about the quality or safety of care in the hospital, please notify the GHS Patient and Family Relations. The department can be reached from inside the hospital at 5-7975 and at (864) 455-7975 outside the hospital.
Please be aware that if you continue to have concerns, you may contact The Joint Commission on Accreditation of Healthcare Organization (TJC) or the Centers for Medicare and Medicaid Services/South Carolina Department of Health and Environmental Control (DHEC).
Obtaining Medical Records
Medical records are available for inpatient stays, emergency room visits and outpatient surgeries from the facility where the service was rendered. To obtain copies of your medical records, you will need to fill out and sign a consent form. The consent form can be picked up in person, mailed or faxed to you.
Copies of medical records can be picked up in one of two Medical Records Office Locations:

There may be a charge incurred with the search and duplication of medical records.
The Processing fee for duplication of records is $25.00.
The Print Fee: Cost per page is $0.65. If there are more than 30 pages, the price is reduced to $0.50 per page, not to exceed $200 per Admission.
If Multiple Admissions exist, the Print Fee applies per Admission
Postal Charges are in addition to the Print Fee
You can receive a copy of your full chart or specify the information needed.
Preferred method of payment is Check. Cash payments are accepted at the Greenville Memorial Office Location. No credit card payments accepted at either location.
Please allow 10-14 business days for your request to be filled. If you have an urgent need, please specify the date needed on the consent form.
