Memory Health Program

The Center for Success in Aging's Memory Health Program offers a comprehensive evaluation which includes medical, cognitive, social and physical evaluations to diagnose memory conditions and develop a plan of care for patients and their family members or care partners. The interdisciplinary team provides excellent care for those faced with the complex needs and changes related to memory changes and impairments.

Adults who exhibit any of the following issues may benefit from a Memory Health Program assessment:

  • Forgetfulness that disrupts daily life (like forgetting a regularly scheduled hair appointment)
  • Challenges or changes in ability to perform daily tasks ( like mixing up a recipe that's been cooked for 40 years)
  • Getting lost when driving in familiar places
  • Difficulty finding words or repeating statements
  • Confusion with time or place
  • Exhibiting  poor judgment (like making uncharacteristic donations to multiple mail requests)
  • Difficulty planning or problem solving (like struggling with hosting a family gathering  at a holiday
  • Misplacing items or finding something in an unusual place (like finding the ice cream in the cabinet and not the freezer)
  • Changes in mood, behavior or personality (like exhibiting paranoia or accusing someone of stealing)
  • Withdrawal from usual activities (like ceasing to attend worship services or shopping for fresh food)

Services Offered

  • Medical Management of mild cognitive impairment, memory loss, or dementia
  • Communication with patient's primary care physician
  • Licensed Social Worker services and other professionals for support, coaching, education and referrals
  • Our Caregiver Resource Library is the gateway to information and resources to help you learn more about being a caregiver, managing stress, providing care, and other pertinent topics related to memory health and self-care
  • Educational and supportive events taught by professionals and experts in the field of memory health and geriatrics
  • Neuropsychological testing and therapeutic counseling (psychotherapy) for individuals and families involved with the Memory Health Program

What to Expect

  • The initial appointment is a 90 minute conference with the Memory Health team which includes: board certified geriatricians, nurse practitioners, licensed, master's-prepared social workers, registered nurses, and certified medical assistants.
  • The patient and family will leave with a plan of care that includes the patient's medical assessment and recommendations along with social worker recommendations and resources that are tailored to the patient and family's non-medical needs. This non-medical support and assessment is one of the unique services available to those served at Center for Success in Aging. Caregiving ABC's is a five session program for caregivers.  Topics include: 

Session 1: Alzheimers, Aging, and Assessment
Session 2: Behaviors, Beliefs, and the Baggage We Bring
Session 3: Communication, Caring, and Coping 
Session 4: Driving, Dangers, and Drugs 
Session 5: Expectations, Emergencies, and Enjoyment
