MD 360 visits is equal to the total number of visits to all MD 360 locations. There are two locations currently, Simpsonville and Greenville/Verdea. Two more are projected to open in the next 12 months. One in Greer and the second in Powdersville.
Our goal is 36,980 by the end of the fiscal year.
Community Care Coordination Program is the coordination and management of strategic patient populations within our community with the goal of decreasing unnecessary utilization and providing better patient care to help facilitate improved outcomes.
Our goal is to enroll 50 high-risk patients by the end of the fiscal year.
