E-Greetings

Create a special, hand-delivered, greeting card for a patient by filling out the form below. Click the button at left to see a large sample of the card, with areas marked that you can personalize. To assure prompt delivery, please enter the patient's full name. Do not use nicknames or husband's first names (example: "Mrs. William Jones" rather than "Betty Jones") when addressing. Greetings received after noon will be delivered the following day.

Please note that greetings received after a patient has been discharged cannot be forwarded to the patient's home address. E-greetings also cannot be delivered on holidays.
