To address the special challenges that come with age, the Center offers a comprehensive
geriatric assessment for those 55 and older. This assessment is a complete medical, cognitive, social and physical evaluation performed by a team of specialists led by board certified geriatricians, the largest group in the Upstate.
Medical, physical, social and psychological problems in older adults often manifest themselves differently than they might in younger patients. Problems are usually related to the combined effects of multiple issues rather than a single cause. The geriatric assessment team approach to diagnosis and care planning reveals those overlapping issues and leads to workable solutions and appropriate plans of care tailored to the individual.
It is important to note that the Center also evaluates seniors who enjoy an active lifestyle, helping them plan for continued wellness. The Center is here to provide a team-based comprehensive geriatric assessment as a service to older patients and their primary care physicians. The Center is not a primary care practice. After each patient's evaluation, he or she returns to the care of the primary physician.
Our team includes geriatricians, a specially trained social worker and a pharmacist with special training in geriatrics. Download referral form
