Breast Health Patient Navigation (BHPN)

Breast Health Patient Navigation Program is offered through a grant from the Susan G. Komen Foundation.

Program

• Help and guidance from nurses
• Clinical breast exams by a surgeon
• Mammograms
• Ultrasounds
• Biopsies
• Follow-up appointments as needed
• Referrals to other physicians
• Education classes for preventive health care

These services are available as needed at no cost to you

Participants …

• Are under 40 years old
• Live in Greenville County
• Have financial need (such as no insurance)

and/or

• Have a family history of breast cancer
• Have questions about breast cancer
