PATH (Partners Achieving Total Health)
Working together for a healthier community
Greenville Hospital System University Medical Center and YMCA of Greenville have come together to create PATH (Partners Achieving Total Health), a new initiative providing access to fitness and preventive care that's both affordable and focused on reaching people before they get sick. This wellness partnership uses community education programs, like Wellness Way, for example, to combat obesity and encourage healthier, more active lifestyles. GHS primary care practices are also providing medical oversight for this particular initiative, with a GHS registered nurse on site delivering care for Wellness Way patients at the YMCA.
Path Program
Together, the partners that make up PATH will provide greater access to health and wellness services, and create opportunities for everyone to succeed on their journey to better health.
Wellness Way
Wellness Way is a medically based, six-week program for adults at risk for chronic conditions such as heart disease. Participants in Wellness Way learn how to reduce their disease by adopting healthy behaviors, including regular exercise and a healthy eating plan. A physician referral is required.
Activate Upstate
Activate Upstate is a free, year-round healthy living awareness program that targets youth and adult obesity and related conditions. The program encourages participants to accumulate at least 30 minutes of moderate-intensity physical activity five days per week. Participants receive information about nutrition, exercise and health education weekly on the Activate Upstate website, and also take part in weekly and monthly weigh-ins and counseling, as well as attend a series of lectures by GHS wellness and medical experts.
Community Education Offerings
GHS physicians anc clinicians will provide health education "lunch-and-learn" and seminars throughout the year at all YMCA branches in the county.
"Power to Prevent" Diabetes Initiative
"Power to Prevent" is currently being piloted at the YMCA- Caine Halter Branch. It will be presented in partnership with the YMCA, GHS and the South Carolina Department of Health & Environmental Control. Other diabetes-focused educational sessions will also be presented at other YMCA of Greenville branches.
PATH Fitness Membership
Two PATH membership options are available: Individual ($65 a month) and Family ($80 a month). Those that enroll during the month of May will not have to pay a one-time initiation fee. PATH membership also includes discounted rates for YMCA-sponsored youth sports and Camp Greenville, as well as nutritional consultations, massage therapy and other services offered at the Life Center.
