Becoming a Patient in REI

If you would like to see one of our specialists, please call 864-455-1600 to make an appointment. New patient visits can be arranged at one of 4 locations, Greenville Memorial Medical Center, Eastside at Patewood, Anderson, and Spartanburg. We encourage patients to bring their partners to the initial visit. New patients are encouraged to have old records faxed to our offices at 864-455-8489. We also ask that the female patient and her partner download and fill out the medical history forms (see PDF files) and bring them with you to your first appointment. We look forward to meeting with you and providing you with the most up-to-date information regarding fertility issues.

To make your visit even easier, we have made the male and female history forms downloadable. This will allow you to fill them out at your convience and bring them to your appointment.
