GHS HEALTH PLAN
- A Preferred Provider Organization (PPO)
- Available to full-time and part-time employees and their dependents
- Two plan options:
- Silver Plan - lower premiums with higher employee out-of-pocket costs
- Gold Plan – higher premiums with lower employee out-of-pocket costs
- Three-tier provider network
- Coverage includes physician/specialist office visits, major medical expenses, outpatient surgery, lab and x-ray services, and prescriptions
GHS DENTAL PLAN
- Available to full-time and part-time employees and their dependents
- Preventive and diagnostic services paid in full
- Coverage includes basic dental services, prosthodontic services, and orthodontic services for participants under age 19
VISION PLAN
- Available to full-time and part-time employees and their dependents
- Nationwide provider network
- $20 eye exam copayment every 12 months
- $20 frames and lenses copayment (up to $120 frame value) every 24 months or $105 contacts allowance every 12 months
HEALTH SERVICES BENEFIT
- Discounted outpatient services for employees and their dependents
SECTION 125
Dependent Care Reimbursement Account – pre-tax contributions to account for reimbursement of anticipated dependent care expenses
GROUP LIFE INSURANCE
- Full-time employee benefit equal to 2 times base annual salary up to $500,000; premium paid by GHS
- Voluntary election by part-time employees for $5,000 coverage
- Covered employees may elect $2,500 dependent coverage
GROUP ACCIDENT INSURANCE
Full-time employee benefit equal to 2 times base annual salary up to $500,000; premium paid by GHS
Voluntary election by part-time employees for $5,000 coverage
SUPPLEMENTAL HEALTH OPTIONS PLAN (SHOP)
- Available to full-time and part-time employees and their dependents
- Voluntary plan providing direct payment to employee for out-of-pocket medical expenses for wellness, physician office visits, lab and x-ray services, prescription drugs, hospitalizations, surgery and related services, at home nursing, emergency room visit, and ambulance services
SUPPLEMENTAL GUL LIFE INSURANCE
- Available to full-time and part-time employees and/or their spouse; child rider to policy available
- Voluntary life insurance plan offering competitive group rates, guaranteed issue provision, portability, and cash accumulation features
VOLUNTARY ACCIDENT INSURANCE
- Available to full-time and part-time employees
- Up to lesser of 10 times employee’s annual salary or $500,000
- Family coverage available; spouse and children’s benefit is a percent of the employees selected coverage amount
- Benefit is paid for loss due to injury occurring within 1 year of accident
BUSINESS TRAVEL ACCIDENT INSURANCE
- All employees covered upon employment
- $50,000 accident coverage while traveling on official GHS business
LONG TERM CARE (LTC)
- Available to full-time employees and part-time employees scheduled to work 17.5 hours per week and extended family members
- Voluntary asset protection plan in case of disabling illness or accident
- Benefits paid directly to participant relating to: Nursing Home Care, Alternate Care Facility Care, Community Based Professional Care, Stay at Home Benefit & Informal Care, Hospice Care, and Temporary Bed Holding Benefit
- Choice of Daily Maximum Benefit options with inflation protection options
PRE-PAID LEGAL PLAN & IDENTITY THEFT SHIELD
- Available to full-time and part-time employees
- Pre-paid Legal Plan – Legal assistance for employee, spouse, and eligible children
- Identity Theft Shield – Free credit report and analysis, continuous credit monitoring; fraud restoration assistance; and $25,000 identity theft restoration reimbursement benefit
LONG TERM DISABILITY INSURANCE
- Coverage provided to full-time employees paid by GHS
- Benefit equaling 60% of monthly base pay begins on 121st day of disability
- Plan requires other income benefit offsets from benefit for workers’ compensation, Social Security Disability, and Early Retirement income
SHORT TERM DISABILITY INSURANCE
- Available to full-time and part-time employees
- Voluntary plan providing benefit up to 50% of monthly base pay (maximum $5,000/month) when disabled due to illness or injury
- Coverage is portable when employment ends
PENSION TRUST PLAN
- Non-contributory retirement plan
- Employees are 100% vested after working 1,000 hours in 5 consecutive plan years
- Normal retirement at age 65 or older; reduced early retirement benefit
RETIREMENT SAVINGS PLAN
- All employees eligible to contribute to 403(b) account
- Voluntary tax-deferred 403(b) savings plan with full-time and part-time employee matching 401(a) employer contributions
- Voluntary tax-deferred 457(b) savings plan
- Non-elective 401(a) plan for private practice and staff physicians
SECTION 529 PLAN “FUTURE SCHOLAR PROGRAM”
- Qualified savings plan under Section 529 of IRS code for post-high school education for tuition, room & board, books, computers and other required supplies
- Contributions and earning grow federally tax-free (state tax-free for SC residents)
PAID TIME OFF (PTO)
- Full-time and part-time employees are compensated for time off for sick leave, vacation, leave of absence, and holidays
- PTO accrues biweekly
